As asked, who has POA because they are the only one authorized to handle her care. You cannot apply for services for her and she may not be able to sign contracts.

In PA they have where you are trained and work for an agency and then you can care for a LO or friend. I would think this is State government funded in some way.

Medicaid may pay for you to care for your friend but it may not be enough hours and will probably be minimum wage. Your friend has to fit an income criteria.

Call your Office of Aging to see what info they have.

Does your friend have a DPoA appointed who will handle or is now handling her affairs? Your profile says she has dementia. If no one is her PoA then getting paid by her in the future will not be legally possible, or she will not have enough funds to pay you. You may want to consider bringing in social services to get her on their radar. She may qualify for some-home services through your state's Medicaid Elder Waiver program. The county will become her guardian and she will get the care she needs, since you will eventually be overwhelmed by them as she declines.

You may want to discuss transitioning into AL on private pay where she will get good care and have more social opportunities with a variety of people. The newer facilities are much nicer than the ones "back in the day". Just need to make sure they accept Medicaid. She will need to set up a PoA or guardianship while she is still able to make those decisions.

You ask your friend to pay you for your caregiving services.
Draw up a contract.
DETAIL what you will do and what the fee is.

If you are asking how you can get paid for caring for her without her paying you directly that is a different matter.
There may be programs through different organizations that if she qualifies for might pay you.
Have her check with the local Area Agency on Aging. Or it is possible that your local Senior Cervices Center has a Social Worker that could help with finding resources.
