Mom has beginnings if dementia. Bedridden after breaking hip last year. Total care. Shes 87. Ive complained numerous times to administrators and then to the state. I dont know states findings . Had family meetings with them. Installed camera to monitor this. They have staff logging when the check her brief however camera doesn't coincide with the staff reporting. Administrators took side of staff saying they did it . Offered to show videos but they weren't interested. We love everything else about this place. Staff are kind and caring in every other way. Most aides change her every 2 hrs or at least check her. They do good for a while then digress. Any ideas to get them to improve in this? I dont want to move her. If I put her under hospice care would that give more help to getting this done ? I asked the director whats their standard of care for changing briefs and was told every 2 hrs. They even asked us to move her once because we complained. I feel afraid to continue complaining for fear of retaliation. But I must advocate for mom . I need some ideas besides moving her

Find Care & Housing
Get help from her doctor. Explain what has been going on. Have her Prinary Care physician send over a medical order stating that based on her immobility and physical condition, she must be re-positioned every two hours and changed if wet or soiled. Ask your medical provider to include educational material about the dangers of skin breakdown if your doctor has such material and agrees to send it. Ask for a copy of the order. Make sure your doctor sends it directly to the medical director at the facility to their specific email address. Give your doctor the name and email address. Ask your doctor to blind copy you on the email. If they ignore physicians orders they are asking for real trouble. See if this strategy finally gets their attention. They cannot ask you to leave if the direction is coming from her physician. They are obligated to comply.

I am not going address the bed sore. About the urine, if she stays still on her back have you thought about the Purewick Urine collection system? It is an external catheter. I have used it 4 times in the ER. I am incontient (can't spell it) and it is a blessing in the hospital. I bought one of their home units, then found I am not a good candidate because: 1. can't seem to put it on myself, and 2. I move too much and I dislodge it. You can see videos about it on YouTube.

I am familiar with the purewick however the nursing home does not have suction to run it . And I'm sure they would just use it as another excuse not to change her. Thanks for the suggestion!

If she is not being touched for 6-10 hours during the day I would move her to another facility. You said she cannot move herself or feed herself, it really sounds like she needs a new placement. FWIW, with my dad (at his second placement) I went and sat 6 hours or so a day during weekdays. Mom went evenings after work and weekends. They had a schedule posted (repositioning every three hours with a diaper check) and we made sure they stuck to it. I don't think Dad ever got a bedsore. The place was so bad it was finally shut down. Mom never tried to get him moved to another facility because the one before was even worse.

You can review CMS survey team findings online. I worked in long term care 14 years. I am sorry you are dealing with this. I would say more but I would sound very negative.

Not everything can be fixed.
Elder care, whether given in home or in facility is often imperfect.
You have a lot of good to say about this place but you are constantly complaining and you are reporting them. And yes, they will eventually tell you "We are sorry your mother and you aren't happy with our care; it may be time to find another facility".

The camera is in place. It is time now to make a study in which you record every single check and ever single change by diary over a two week period. This is going to have to be set in stone, then set before the administrator.

To whom are you reporting? Have you contacted APS to find out the correct agency? Have you contacted JCAHO? Have you contacted an Ombudsman and discussed with admin, asking for followup?
What repercussions of this have you noted on loved one's skin condition?

As to Hospice? Unlikely to help in this situation as they are physically "there" only twice or three times a week for bedbaths. But Hospice in every other way may give added voices, skin observation, and support. If your Mom is at end of life and her doctor will ask for end of life care, and you understand all this means in terms of treatment, etc. it may be a good thing to consider.

I agree. At this point it’s time to go over their heads and complain to the officials that will show up within 48 hours to check on the resident. ALL OF THOSE suggestions are great. Hopefully they’ll take it into consideration because the staff won’t change until administration’s hand is forced.

I’d move Mom to another nursing home .
Sounds like they don’t have enough help maybe .
Hospice only sends an aide a few times a week for baths . It won’t make the aides at the nursing home check Mom more often .

The nursing home and the state surveyors stated the standard of care is briefs are to be checked every 2 hours . I was ok with 4 hours but 4 became 6 and 6 became 8 etc. She has gotten sores and uti. That's why

Actually changing the brief every 2 hours may not be necessary.
If it is clean, dry there is no need to change it.
What is important is getting her up every 2 hours. Getting her moving every 2 hours.
If they are checking the brief to see if it is soiled they are also checking skin condition.

If she has other conditions that would qualify her for Hospice then putting her on Hospice is a great idea BUT.....
She will still have the same staff and the staff will still do what they do.
The things that will change is...
The Nurse from Hospice will check her weekly.
The Hospice CNA will be the one giving her a bath or a shower NOT the facility staff and they will stop doing showers or baths. (Medicare will not accept billing from both for the shower or bath)

If this is the ONLY problem that you have with this facility I think you are lucky that mom is being cared for and that the staff is kind, caring.
Now if mom has skin that is irritated, rashes, if there is a strong urine odor on her or her clothing or bedding then you may have reason to raise a stink (pun intended)

I should have clarified. Im asking them to check her brief every 2 hours which will change her position. She doesn't move on her own. If its dry then no change needed. Shes going 7 to 10 hours without being checked or turned consistently. She has a sore in her bottom. She has uti. She requires bed baths etc. She does not move in her own. The state of Texas standard is every 2 hrs . The nursing home told me their standard is also 2 hours for turning and checking a brief. No one offers her a drink. She will feed herself wirh her hands as she is shaky. She has dementia that has manifested in not wanting to move because she fears falling .
