I’m looking at agencies for 24-care at home. The agency wants to do a needs assessment and psych evaluation, before they decide which caregivers would fit. Makes sense.



But can anything bad happen by doing the evaluations? And does it go on the medical record?

Whether is goes beyond the agency's records probably has more to with whether they are hooked into a system like MyChart for the same system that her doctor/hospital uses.

I would ask the agency to whom they would share this evaluation. It would need to be someone with explicit HIPAA authorization.

FYI, I asked. All the agencies told me, the assessment is only for their internal use. It’s just so they can identify who are the best caregivers for the elderly person.
