She has never been a pleasant, caring person, yet I am her only child and cannot leave her care to a nursing home that does not care for residents whose families are not " watchdogs". Cannot deal with this roller coaster much longer. She has even begun calling some of her nurses and aides inappropriate names and digging her nails into one.

Babalou, pain as a result of being in bed for almost 2 years from broken kneecap, for which she refused surgery or therapy combined with dementia, mini strokes, and seizures. She is 92 and even though this time she is only eating a tablespoon or two at each meal and getting honey-thickened nectar she is bouncing back just enough to be pulled out of hospice comfort probably in a few weeks.

Can you ask for palliative care? Different from hospice, but I believe they would do adequate pain relief.

Is she on any psychiatric meds for aggression?

Yes, Celexa did not help. Now Halidol (spelling?) which is calming her in evening and night. She still sees little children, says there are pins in her finger, or fingernails, says she gets up and walks and CONSTANTLY asks for water. Speech therapist only allows puréed food and only sips from a teaspoon every few minutes. She gets morphine for pain and fentanyl patch as well.

Up the morphine if she still has pain. Up the Haldol too if it helps. It sounds like not enough pain control. Sorry you are going through this.
