If not where can I go to have it changed?

Medicare will transfer state to state. MediCAID has to be re-qualified in the new state and may have different income requirements in new state. Check with Medicaid office in VA to determine application process.

Thank you guesthopadmin! So I don't need to do anything with Medicare? My mom can use the same card etc... in VA even if she's from TX?

Your mom will need to let Medicare know she moved, so they don't think her account has been compromised. You might check to see if she enrolled in traditional Medicare or one of the state Medicare "alternatives" to get extras like glasses or dental or prescriptions. If she has a special version of Medicare/supplemental your mom needs to make sure new state honors her Medicare if it's not traditional. Give Medicare a call and they can help in transition. You or she need to make sure any new doctors are accepting new Medicare patients in the new area.
